EnerJet Acne Scar Treatment

At Carniol Plastic Surgery, we offer EnerJet: The Needle-Free Revolution for Rolling Scars and Icepick scars. EnerJet utilizes Jet Volumetric Remodeling (JVR) technology. Instead of needles or heat, it fires a high-speed jet of therapeutic solution (typically hyaluronic acid and platelet-rich plasma) deep into the dermis.

  • The Blast Effect: The kinetic energy creates a “controlled micro-trauma” that breaks up the tough, fibrous bands pulling the skin down.

  • The Volume Effect: The solution spreads laterally, instantly hydrating the area and “tenting” the skin upward for immediate visible improvement.

  • The Long-Term Effect: This process triggers a massive healing response, creating new collagen and elastin that permanently smoothens the skin surface.

Platelet-Rich Fibrin Matrix (PRFM) / Platelet-Rich Plasma (PRP)

Harnessing the body’s own growth factors, our doctors can further improve individual scars. By drawing a small amount of blood from your arm and extracting the growth factors, we utilize your platelet-rich plasma to promote scar healing. This treatment can be combined with laser acne scar treatment and microneedle radiofrequency therapy or performed as a standalone option. Treatment sessions can be repeated every 4-6 weeks, focusing on targeted scars.

Subcision with Filler or Fat for Acne Scars

Stubborn acne scars that are firmly attached to deeper tissues can be challenging to elevate. Our facial plastic surgeons perform subcision, a technique involving the use of a fine needle to separate the scar from the underlying tissue, resulting in immediate elevation of atrophic scarring. Facial fillers, such as Juvederm, Restylane, or Versa, are used to prevent the scar from tethering again. Alternatively, liposuction-fat grafting, where fat cells from the belly or thighs are transferred to improve acne scars, can be performed in our office. Individual scars are addressed during specific sessions, making this treatment ideal for severe acne scars.

Laser Treatment of Acne Scars & Fractional

Laser therapy stimulates collagen and elastic fibers in the skin, resulting in tighter and less deep scars. We utilize fractional CO2 lasers to target and treat scarred areas, effectively blending acne scars with the surrounding skin. The procedure is virtually painless, with minimal downtime. After, you can resume your daily activities after the treatment sometimes the next day. Over time, your skin will produce more collagen, and we know that patients will continue to see improvements even 3 and 6 months after treatment. For enhanced results, we may combine the fractional CO2 laser with Erbium laser treatment. Laser acne scar treatment not only addresses scars but also improves signs of facial aging, dark spots, and offers some of the benefits of a non-surgical facelift.

How is EnerJet different from Microneedling? While microneedling creates vertical channels with needles, EnerJet uses kinetic energy to subcise the scar with a quick “blast” under the skin. This allows Dr. Carniol to treat the underlying cause of icepick scars—the tethered bands—much more effectively than needles can, all without heat or actual needles.

How is EnerJet different from TCA Cross? TCA Cross involves using a needle to insert a “superficial chemical peel” into a deeper layer of the skin to make the skin respond. This can be effective but is very irritating to the skin. EnerJet using a microdroplet of saline and filler to provide instant improvement in the skin, and by the kinetic energy, stimulates the body to continue to improve over time! TCA Cross carries risk of postinflammatory hyperpigmentation, so it can be difficult for patients with darker skin types (higher Fitzpatrick types).

Is EnerJet safe for all skin types? Yes. Because EnerJet is a no heat, no energy treatment, it is exceptionally safe for all skin tones, including darker skin types (Fitzpatrick IV-VI) that may be at risk for hyperpigmentation with traditional lasers. That means that for many acne scar patients that are African-American, South Asian, East Asian, or Middle Eastern, we have an excellent option for their acne scars!

How many EnerJet sessions are needed for acne scars? Most patients see a visible “lift” after a single session, but for optimal, permanent scar remodeling, we typically recommend a series of 2-3 treatments spaced 4 to 6 weeks apart. One of our patients shared that she inputted her photos into ChatGPT and it told her that her scars had a 35-40% improvement in a single treatment!

After the EnerJet treatments, most scars are left very superficial, making them great for microneedling!
